We pleased to announce that we have amended our Risk Stratification tool so that it supports the Unplanned Admissions DES 2014/15. This means that the patients in the ‘very high risk’ ┬ásection are the top 2% of adult patients (aged 18 and over) of your Practice’s registered list.

We have also changed to using the Nuffield Combined Predictive Model instead of the Kings Fund Model. The Nuffield Model is as effective but delivers an absolute risk score instead of a relative risk score. This means we can provide you with a percentage risk for each patient or 1 or more emergency admissions in the next 12 months.
